THE WARNING
CARD:
NOTIFICATION TO STUDENT OF UNSATISFACTORY PROGRESS

A warning
card is issued for any nursing student who is doing less
than satisfactory work in a School of Nursing course, or who
is not meeting essential qualifications for continuance in
the program of study. A warning card must be used for those
students who are in danger of failing or who have less than
a 2.0 grade (undergraduate students) or 2.7 grade (graduate
students) at mid quarter.
The instructor meets with the student to discuss the student's
progress and any concerns, and prepares a memo outlining expectations
and conditions the student must meet to complete the quarter
successfully.
After the discussion, the student is asked to sign the card.
Student signature indicates that that the student has seen
the card and is aware of the concerns. Student signature does
not necessarily indicate agreement.
The signed warning card is sent to Academic Services where
it is placed in the student's file. All warning cards are
removed at the time of graduation and do not become part of
the permanent file.
BSN students are encouraged to schedule an appointment with
the Undergraduate Advisor to discuss the situation. Graduate
students may schedule an appointment with the Assistant Dean.
All students are eligible to receive support through Academic
Services and/or other sources.
